Academic Accommodation Consent Form
Fill out this form if you’re approved for academic accommodations and are requesting access to them for an upcoming semester. Filling out this form requests your accommodation letters for any given semester.
Test Relocation Form
Fill out this form if you’re approved for testing accommodations and need to take a test with SAS.
Alternate Format Text Request Form
Fill out this form if you’re approved for alternate format texts and would like us to procure texts in an alternate format for your classes.
Release of Educational Record
Fill out this authorization form if you’re registered with SAS and need access to your documentation on file and/or a letter describing your approved accommodations to give to someone outside of SAS.
Sharing Residence/ESA Roommate Agreement
Fill this form out if you’re approved for an emotional support animal (ESA) and will be living with roommates who you haven’t lived with in the past. After you and your roommate(s) fill it out, please send it to SAS.
